Completed forms must be sent to: RI Dept. of Labor and Training, Professional Regulation Unit, PO Box 20247, Cranston, RI 02920-0943. Forms will be processed on a first come, first serve basis.

Exams are accessible to the handicapped. Individuals requesting interpreter services for the hearing impaired must notify Professional Regulation at (401) 462-8580 at least three business days in advance of the examination date.
