Temporary Disability Insurance
Using your Electronic Payment Card

 

To activate your Electronic Payment Card and to select both
your four-digit Personal Identification Number (PIN) and six-digit access code,
call Chase Customer Service: 1-866-847-8586

You can use your Electronic Payment Card to withdraw cash at over 900,000 Automated Teller Machines (ATMs) worldwide, with surcharge-free access at over 40,000 Washington Trust, Allpoint and Chase ATMs in the U.S. Just look for ATMs that display the logos on the back of your Card and:

1.

Insert or swipe your Card.

2.

Enter your 4-digit PIN on the keypad.

3.

Press “Withdrawal” and then “Checking.” Follow the instructions until the transaction is complete.

4.

Take your cash, your Card and your receipt.

This Card CANNOT be used until you call to activate it and sign the back.

Use your Electronic Payment Card instead of cash or checks to make purchases at merchant locations anywhere. These locations include department stores, grocery stores, restaurants and more! Just swipe your Card through the Point-of-Sale (POS) machine or present the Card to the cashier. Then sign the receipt (or enter your PIN on the POS machine keypad).

You can also use your Card to place phone or Internet orders; just provide your Card number and the expiration date.
