Frequently requested information:

  • If you do not wish to file your claim online, you may download the "paper" application at www.dlt.ri.gov/tdi/TDIinstructions.htm or;

  • You may request a paper application by calling (401) 462-8420 and pressing option #1; once you provide the required information, TDI will mail you a paper application.

Most likely, your submission did not include answers to all required fields. You may try to re-enter a complete claim using our online filing system. Alternatively, you may request a paper application (see above).
  • It takes three to four weeks from the date we recorded your claim to obtain the medical certification required so that TDI staff may determine your eligibility and process your claim for benefit payments.
  • If your doctor’s office has sent TDI information that is not currently reflected in your current claim status, please alert us and we will follow up directly with your doctor’s office.
  • If you experience an interruption in your weekly benefit payments, it may be because your original certification period has expired, and TDI is awaiting additional information needed from your doctor in order to extend your certification period.
