Unemployment Insurance change of address forms due
1099G forms to be mailed to all UI claimants by end of January

Any Unemployment Insurance customer who received benefits in 2009 AND whose mailing address has since changed must file a change of address form with the RI Department of Labor and Training by January 18 in order to receive an important form needed to file a 2009 tax return.

This form, known as 1099G, provides a customer with an annual accounting of unemployment benefits. In Rhode Island, all unemployment insurance (UI) benefits are fully taxable and must be reported with a customer’s annual tax return. The change of address form is available online at the department web site, www.dlt.ri.gov. Also, beginning Wednesday, Jan. 14, advertisements featuring a returnable version of the form will run in a number of local newspapers, including The Providence Journal.

The annual 1099G forms for all 2009 unemployment insurance claimants are slated to be mailed at the end of January.
